By checking this box, the parent permits Miss Tilly's Ballet & Theater Arts to record photographs or video of your child as a program participant. You grant the Miss Tilly's Ballet & Theater Arts all rights to use these sound, still or moving images in any medium for educational, promotional, advertising or other purposes that support the mission of the Miss Tilly's Ballet & Theater Arts. All rights to the photographs or video belong to the Miss Tilly's Ballet & Theater Arts.

The non-refundable tuition deposit of $250.00 reserves a space for your child until September 1, 2015 when the tuition balance is due.

In the 2 and 3 year-old divisions, the $250.00 represents a trial period fee and the balance of tuition is not due until October 22, 2015.

After you fill out the registration form, you will receive an email confirmation with your child's class details within a few days.
